Building your carrier packet is quick and easy! By you completing this signup page, you are the Admin for the company and can legally agree to our terms and conditions. We recommend you have the following information before continuing:

  1. Your company contact information
  2. A copy of your company’s Carrier Packet
  3. All forms that your carriers are required to agree to (i.e. Customer Carrier Agreements, Worker's Comp Waivers, Quick Pay Agreements, etc...) ALL OF YOUR FORMS MUST BE UPLOADED AS ONE DOCUMENT. You cannot upload multiple files.
  4. A credit card or ACH info (Bank Login or Routing/Account Number)

Keep in mind, you can update or change any and all data and/or forms later when you login. In addition to making changes, you will be able to add secondary users who will be able to send carrier packets and perform basic tasks.

Only you will have the ability to make changes to your company’s Online Carrier Packet.

The price for your MyCarrierPackets.com service is a flat rate starting at $394 per month and is based on your company’s gross annual revenue. After your first free month of service, your credit card or bank account will be charged monthly or annually until you cancel your service. You will not be charged until after your free month of service. If at any time during your subscription you decide to cancel service, you will have the ability to download all of your carrier packets as individual pdfs prior to cancelling the service. It is your responsibility to download the packets, as they are not automatically sent to you upon cancellation. If your subscription is cancelled by Assure Assist, Inc. for non-payment, you will need to bring your account current in order to download your data and images. Only the Admin (main user) has the ability to cancel service and/or download "All" of your company's carrier packets. There are no refunds once payment has been processed. If you are not able to process cancellation through the subscription management feature on the website, you must contact us at accounting@assureassist.com or (818) 453-8591 to cancel.
